You request A1 nursing bureau to provide service on behalf of the patient, at home or at another location approved by A1 nursing bureau. You agree that the service is provided with advice from a legal practitioner and that you assume responsibility for the service and the risks involved. A1 nursing bureau will ensure a high quality of service but will not be responsible for any service errors including criminal activity by caregivers. You understand and agree that the service provided by A1 nursing bureau directly or through the careers/assistants of A1 nursing bureau are risk of failure and/or adverse effects that potentially result in death or permanent disability it is possible you understand and agree that A1 nursing bureau does not guarantee any recovery or results from the services provided. You agree to provide a safe environment for the caregivers to discharge their duties and you will ensure that they are not physically or emotionally abused, abused or victimized in any way. A1 authorizes nursing bureau to collect patient data and use/share it with others (without disclosing patient’s identity). A1 nursing bureau will not be responsible for misuse of patient data by caregivers, aides, employees or other party.
We want to warn you that when any of our staff goes on duty at your home, after checking their bags daily, you should allow them to go inside. And when any of our staff leaves home after completing duty, then after checking his bag daily, let him go out of his house. If any client does not do this, and later their theft or loss of anything happens, then A1 nursing bureau will not be responsible but the client will be themselves.
Dear customer, we would like to inform you that, if you do duty with our staff without informing company a1 nursing bureau, then there will be a police case against you.
